Data We Collect

Stockport NHS Foundation Trust is the data controller. We are using MS Forms to collect feedback from our participants on how they are progressing through our ePMP and measure satisfaction.

We use this data for five purposes:

Track participant progress:     By completing your questionnaire at the end of each module, your ePMP coach, Pain Management Consultant and you can better identify how your knowledge and skills in pain management are progressing. We can also identify any problems you might be experiencing either accessing or benefiting from the ePMP.

Measure participant outcomes:    This is important for ensuring you have benefitted from the ePMP and to help us plan your next pain management steps with you.

Improve the ePMP:     Stockport Pain Management Service adopts a constant improvement approach to ensure we offer ePMP participants the best possible learning experience and results.  We also want the ePMP to be helpful to everyone, including people with equality, diversity and inclusion needs and qualities. We value your feedback and use it to help us identify where the ePMP is working well and where it can be changed and improved.

Demonstrate the effectiveness of the ePMP:      Stockport Pain Management Service’s ePMP represents a new innovation in pain management education and support. The team, our managers and stakeholders have invested time, energy and money into the project. In order to keep the investments going, we are required to demonstrate whether the ePMP is effective and welcomed by the patients and their families we serve.

Storage and Destruction

The answers to your questionnaires along with any supplemental information by email or otherwise communicated will be  controlled by Stockport NHS Foundation Trust’s Pain Management Service. Data is stored on our NHS servers, which fulfil NHS and UK standards for privacy and data protection.

We will retain your data for no longer than is required for the purposes of the ePMP, and for your medical records in line with NHS data protection policies. For further information and how to access your data, please visit: https://www.stockport.nhs.uk/IG/ViewPage.aspx?p=3001
